To access your Elation Passport, you must register first. To register, please contact your WellAdvantage care team and follow the steps below. If you are registered, click the button to sign into your Elation Passport account.
Elation Passport allows your provider to send messages to your Passport like an email. You get notified by email or text to check your Passport for the new message from your provider. From Passport, you can read your provider's letter and view any attached documents- like lab results- and if you have a question, reply to the letter right there in Passport. Any message you write in Passport is returned to your WellAdvantage provider.
You may also access your health information, such as allergies, current medications, etc. Notes from each WellAdvantage visit will automatically be available to you online.
Contact the WellAdvantage care team to set up your account. They will send you an invitation to activate your account. Patients are required to provide an active email account where the invitation will be sent, and patients must provide a cell phone number where an activation code will be sent. If you are unable to provide an email or cell phone number, please speak with your care team to discuss an alternative option to set up your Elation Passport account.
